Veterans Inc. Regional Health Care Navigator in Worcester, Massachusetts

We are seeking to fill the Health Care Navigator position and add valuable members to our team to accomplish our mission!DAILY RESPONSIBILITIES:Integral team member of the Supportive Services for Veterans Families (SSVF) Regional Team. This team rapidly rehouses at-risk or homeless veteran households while providing case management to identify and overcome barriers that could result in a housing crisis.This specialized position receives internal referrals from the New England SSVF Team, referrals have identified healthcare barriers or needs. Clients can reside in any of the 6 New England states.Working with veterans heavily utilizing telecommunications due to the regional nature of the position. Must be comfortable frequently utilizing the following tools: phone, email, and Zoom.Reoccurring tasks include simplifying enrollment into healthcare (Private, State, or VA), locating a PCP, assistance with scheduling appointments.Networking and building connections with health care teams and homeless communities to streamline treatment to our veterans.Assisting veterans with overcoming the common barrier of lack of transportation and/or childcare to increase follow through with scheduled appointments.Provides education on wellness-related topics. Such as smoking cessation, diabetes management, and or exercise. As well as linking veterans with local or VA support groups.Eliminating the need for homeless veterans to visit emergency rooms to receive care. Educating them on how to use their healthcare coverage properly.Responsible for helping the veteran gather documentation or completing paperwork for enrollment or other healthcare-related needs.Assist with finding care for major health problems, such as PTSD, Major Depressive Disorder, Substance Use Disorder, and Cardiovascular disease, and other chronic illnesses.Following up with veterans to ensure that have continued care and assess if additional assistance is needed.WHAT YOU MUST HAVE TO BE SUCCESSFUL:Bachelor's degree in Human Services, Social Work or other related field required.Military experience could be substituted for a Bachelor's Degree.Minimum of 2 years of Case Management, Social Work, or transferable experience.Computer proficiency in Microsoft Office Suite.Must be able to pass a CORI and background investigation.WHAT WE CAN OFFER YOU:Comprehensive Benefits Package, which includes:BCBS Medical, Dental, and Vision InsuranceEmployer Paid Short and Long-Term Disability and Life Insurance.$2000 Medical Opt-Out program if you have medical coverage through another source.Retirement Plan (403B) with a $2000 MatchFlexible Spending AccountsTuition Reimbursement ProgramPaid Parental LeaveiMailed, emailed, or faxed cover letter, resume, and applications will not be reviewed nor considered!If you need assistance completing an online application please contacticareers@veteransinc.org.
