PRN Registered Nurse Behavioral Health

As a Behavioral Health RN, you will:

  • Patient Assessment & Care Planning: Evaluate patients and families to identify physical, psychosocial, and environmental needs. Develop, implement, and revise individualized plans of care based on ongoing assessments.

  • Patient Visits: Conduct initial evaluations and follow-up visits, addressing patient, family, and facility needs.

  • Communication & Reporting: Notify supervisors, physicians, and team members of significant findings, changes in health status, or unsafe conditions.

  • Education & Counseling: Provide education on behavioral health philosophy, symptom management, and care plans to patients, families, and facility staff.

  • Team Collaboration: Supervise and document care provided by behavioral health aides and LPNs.

  • Problem-Solving: Address facility issues and report concerns to supervisors as needed.

Qualifications:

  • Current, unrestricted state RN licensee

  • 1+ years of experience in a clinical care setting as a nurse

  • Able and willing to travel within branch/office coverage area.

  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.

  • Must be able and willing to travel 50%

  • Associates Degree or Bachelors Degree in Nursing is preferred

  • Previous Home Health or Hospice Experience is preferred

  • Previous experience with Homecare HomeBase is preferred
