Meryl's Safe Haven Case Manager in Worcester, Massachusetts

Department: Family Housing & Shelter Report to: Chief Operating Officer Classification: Full Time, Exempt Work hours: 35 hours per week (Weekdays) Hourly Rate \$22/hour + benefits Position Overview: Reporting to the Chief Operating Officer, the Case Manager is responsible for the case management of families in a congregate emergency shelter site in Worcester. The Case Manager will provide and guide families through holistic support while assisting families overcome barriers to becoming self-sufficient. Key Job Duties and Responsibilities Include: Supporting Families \ Complete intake and needs assessment with families, which may involve a broad rangeof areas including housing, finance & budgeting, employment, education, food &nutrition, school & daycare, legal issues, health, parenting, and daily living skills \ Interview and evaluate families to formulate Individualized Service Plans based onProgram requirements \ Work closely with families to identify and build upon strengths and develop strategiesto address barriers and concerns identified through the assessment process \ Help families build self-sufficiency plans \ Assist families in transitioning to self-advocacy roles \ Advocate on behalf of families and attend administrative hearings, if necessary \ Perform home visits and/or unit inspections \ Establish, evaluate, and monitor goals for families linked to measurable outcomes \ Work collaboratively with an interdisciplinary team of collateral providers and partners to ensure the coordination of services \ Maintain the structure and policies of the program through orientation of families byreviewing program rules, policies, and case management visits \ Maintain accurate case records by documenting all services rendered including up-to-date case notes, telephone contact logs, and referrals \ Enter required data into the agency databases with reporting, as required \ Provide emergency response management to address any crises or safety issues thatmay arise \ Accompany and/or transport participants to appointments as necessary \ Assist families with arranging appointments \ Perform other related duties as assigned Knowledge, Skills, and Competencies: \ Knowledge and experience working with low-income, culturally diverse, and at-riskfamilies \ Demonstrated knowledge of community resources available to low-income individualsand families \ Excellent customer service, organizational, oral, and written communication skills \ Ability to manage multiple priorities and work in a fast-paced environment \ Proficient computer skills required in basic computer software and data collectionsystems \ Crisis management skills \ Ability to maintain accurate records and confidentiality of information \ Excellent attention to detail Minimum Qualifications: \ Bachelor\'s degree preferably in a Human Services related field (significant casemanagement experience may substitute for a degree) \ Three to five years experience working with economically disadvantaged youth and/orfamilies \ Minimum of one-year assessment, advocacy, and case management work in a relatedfield required \ Experience working with populations with mental illness, developmental disabilitiesand/or substance abuse, and economically disadvantaged individuals and/or families Preferred: \ Valid driver\'s license \ Bilingual (English/Spanish, English/Haitian Creole, English/Portuguese) Physical Requirements: \ Prolonged periods of sitting at a desk and working on a computer \ Prolonged periods on-call at site \ Must be able to lift up to 15lbs at a time Employee Compliance Requirements \ Comply with all Compliance regulations \ Adhere to the components of the Compliance Program and annual trainingrequirements \Understand that annual training is a required condition of employment \Ensure job tasks are performed in a legal and ethical manner \Actively assess the work area for non-complianc issues and notify the supervisor
