Community Healthlink Advanced Practice Registered Nurse in Worcester, Massachusetts
Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a part-time (20 hrs/wk) Advanced Practice Registered Nurse in Worcester.
The Geriatric Team provides behavioral health services, including psychiatric consultation, to nursing homes in Central Massachusetts and MetroWest. Staff include licensed independent clinical social workers, licensed mental health counselors, psychiatrists, advanced practice psychiatric nurses, and administrative support staff.
The Psychiatric Clinical Nurse Specialist is responsible for providing psychiatric consultation services to patients of the Community Healthlink Geriatric Services.

Complete psychiatric assessments, medication consultations, and follow-up services consistent with the advanced practice agreement with the supervising M.D.
Provides each patient, in collaboration with the clinical team, a comprehensive assessment, diagnostic formulation, problem list, treatment plan, and ongoing patient care, further evaluation, consultation and collaboration as necessary to address the safety and treatment needs
Provides recommendations for medication to nursing facility medical staff.
Works collaboratively as a member of patient-centered treatment teams
Maintains all documentation for services provided, with review of information from other providers in the EMR
Works in the nursing facilities and rest homes served by the Community Healthlink Geriatric Services.
Regularly attend required program meetings
Prepare and maintain confidential client documentation
Demonstrated knowledge base of senior adults
Demonstrated current knowledge of psychiatric medications
Ability to work as a member of a team
Ability to accurately assess client's presenting concerns
Ability to effectively communicate with clients, peers, and other professionals
Timely and accurate documentation skills
Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
Certified by the Massachusetts Board of Registration in Nursing as a Psychiatric Clinical Nurse Specialist
Current DPH and DEA Certificates Experience working with a geriatric population
Ability to work as a member of a team
Must be able to pass a CORI background check
Current valid US-issued drivers license and ability to provide a registered and insured automobile for work related purposes including travel between work sites
