Actalent Full Time LPN - Behavioral Health in West Springfield, Massachusetts

Seeking a caring, energetic, optimistic, and reliable Licensed Practical Nurse to join an organization that build's a world in which individuals

Job Description:

  • Conduct patient history intake and preparation

  • Collect and manage lab data

  • Conduct advanced telephone triage functions and participate in research activities

  • Act as a liaison between patients, families, and other healthcare professionals

  • Maintain accurate records and documentation

  • Monitor and order supplies

  • Perform quality assurance (QA) functions as directed

  • Assist with the orientation and training of new personnel and clinical staff

Qualifications:

  • State of MA LPN License

  • Associate’s degree with LPN certification and 1-2 years related experience.

  • One year of supervisory or human service experience is desired.

  • Knowledge of and respect for diverse populations.

  • Must be able to travel within the region to access residential settings, employers, and community organizations.

Schedule: Monday-Friday 8:30am-3:30pm. NO WEEKENDS
