State of Massachusetts Acquired Brain Injury/Moving Forward Plan Human Services Coordinator I in Waltham, Massachusetts

The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivated and talented individual for the Acquired Brain Injury (ABI) and Moving Forward Plan (MFP) Human Services Coordinator I position to function in the Metro Regional Office. This Human Service Coordinator I position will be coordinating services for individuals eligible for ABI/MFP Residential waivers pursuant to Hutchinson settlement agreement. This Human Service Coordinator I is the primary contact for individuals and families served by the Department and is expected to effectively represent the Metro Regional Office by building respectful and collaborative partnerships with individuals, families, guardian, provider agencies, nursing facilities, and other stakeholder groups involved in the work of the Region and the Department. _Duties and Responsibilities (these duties are a general summary and not all inclusive):_ * Leads an Interdisciplinary Team in the development, coordination, and integration of services plans for assigned individuals. * Facilitates meetings with the interdisciplinary team to develop Transition and Individual Service Plans (ISPs). * Maintains and updates written and electronic records. * Advocate on behalf of the individual. * Monitors and documents the provision of services and modifies service plans as needed. * Protects the privacy of information. * Ensures that the individual's legal, civil and human rights are protected * Provides information and other services to individuals and their families. _ Please Note:_ * /Driving will be a necessary part of this position./ * Schedule: Monday - Friday, 8:30AM to 5:00PM Off: Saturday and Sunday * Interested applicants should be flexible in schedule as there may be occasional nights and weekend meetings with individuals, guardians, families and other agencies. _Preferred Qualifications:_ * Knowledge of the principles and theories of individual and family growth and development. * Knowledge and understanding of acquired brain injury and related clinical considerations. * Knowledge of the types and manifestations of development, psychological and emotional disorders for individuals with an acquired brain injury. * Knowledge of interviewing techniques. * Ability to communicate effectively through verbal and written means. * Sensitivity to religious, ethnic and linguistic diversity of individuals with intellectual disabilities/developmental disabilities and their family members. * Ability to evaluate and maintain accurate records. * Ability to deal tactfully with others, and to establish and maintain harmonious working relationships with others. * Ability to demonstrate skill in handling confidential information. * Ability to prioritize, respond to multiple demands, and manage time efficiently. * Ability to seek guidance and support and take direction from supervisors and clinical consultants when needed. * Applicants should be comfortable with a personal computer as there will be daily entries in specific data bases, HCSIS, Meditech etc. About the Department of Developmental Services The mission of the Department of Developmental Services (DDS) is to create innovative and genuine opportunities for individuals with intellectual and developmental disabilities including Autism Spectrum Disorder to participate fully and contribute meaningfully to their communities as valued members.
