Project Health Care Coordinator The G?ndara Center provides residential, mental health, substance abuse, and preventive services for children, adults, and families across the Pioneer Valley and eastern parts of Massachusetts. We are in search of an Organized Care Coordinator committed to providing services for individuals with serious mental illness for project health initiative. The Care Coordinator provides integrated coordination and cares for infectious disease-related diagnosis (i.e. HIV and Hepatitis C) as an active member of a multidisciplinary team working in coordination with therapists, psychiatrist(s), and infectious disease partnered provider(s) Duties Actively facilitates the coordination of care for an assigned panel of BH patients across the continuum of care through collaboration with the patient, family, and health care providers in achieving optimal patient outcomes. Serve as a link between the consumer, the infectious disease provider, and specialists. Provide HIV and Hep C coaching, education, and care coordination for all Gandara clients in Project Health Program Participate in Project Health staff conferences, case reviews, and team meetings Monitors the participant's progress using reporting tools, extracting data from the participant medical records and monitoring adherence to medical and behavioral treatments. Complete SAMHSA required documentation with clients timely (enrollment and 6 month reassessments) Coordinating care with the hospital, ER, consulting physicians, and community resources Facilitate and organize team integration meetings for coordination of care. Coordinate rapid testing, documentation, and information sharing with Gandara staff and EHR Develops and documents workflow for the PCMH delivery model and the reporting of outcomes. This includes: Participating with the project team and performance improvement staff to develop and document workflows and protocols for the delivery model Required Skills Qualifications/Requirements 2-3 years' experience in behavior health or primary care setting High School Diploma or Associate's Degree in a related human service, psychology, counseling field required; Bachelor Degree preferred. Knowledge of patient centered care and/or integrated care Proficient in Microsoft applications and Electronic Health Record (EHR) Demonstrated understanding of client/family driven treatment and the wrap-around approach to services.
