Registered Nurse Essential Functions:
Conduct regular in-person visits with client and caregiver to ensure clients remain safe and healthy in the community while decreasing/preventing unnecessary hospitalizations and re-hospitalizations
Ongoing evaluation of client's health status, safety in the community, potential resources needed and evaluate program eligibility. Including formal semi-annual and annual assessments
Select, train, evaluate, and supervise AFC/GAFC caregivers in conjunction with the care manager
Complete required assessment documentations including: Health Status Form, Minimum Data Sheet, Multidisciplinary Care Plan and other documentation as required
Work collaboratively with the interdisciplinary team to create, monitor and implement client-centered care plan
Completing a nursing progress note corresponding with each encounter, or more often as the member's condition warrants
Report observations and functioning status of the clients in the internal charts and report changes in the health status of any member to the member's PCP
Contact with client's primary care physician to obtain medical information and physician clearance; also reporting any changes in medical status
Performs any other additional duties that may be required by management at any time
Registered Nurse Requirements:
RN Massachusetts license required
Fluency in Spanish required**
Case management and/or nursing education experience strongly preferred
Experience with Minimum Data Sheet (MDS) and MassHealth Programs preferred
Minimum of 1 year experience in an acute or home health care setting or comparable work experience recommended
Past experience working with behavioral health population is a plus
