Care Coordinator Location - 395 Liberty Street, , Springfield, MA, 01104, United States Job Category - Health Care, Nonprofit - Social Services Employee Type - Full-Time Minimum Experience - 2 Years Contact information Name - Jennifer Cuffy-Ramos Email - jennifer.cuffy-ramos@bhninc.org Description The Care Coordinator for the One Care Program provides high quality, community-based care management services, as well as provider, family, and collateral collaboration and consultation on assigned cases. Through the coordination and integration of care, the Care Coordinator assists individuals ages 21-65 with medical, behavioral health, and/or substance abuse concerns in navigating the health care system and improving their health outcomes in addition to addressing social determinants of health needs. Your experience, knowledge and great skills will support the OneCare program with the following: Providing telephonic and in-person Care Management and Care Coordination support to members based on the collaborative spirit of the integrated health model that promotes improved health, independence, and self-sufficiency. Providing outreach services to members in the community (i.e., member homes, health clinics, collateral providers, supportive services, other community settings) as needed and/or in accordance with CCA program requirements, helping them to connect to needed resources and to overcome barriers impacting their care. Providing transportation for members in personal vehicles as needed (i.e., when there is a critical need that significantly impacts a member?s medical or social determinants of health status and CCA transportation and/or BHN vehicles are not available). Acting as primary ?point of contact? for all members on caseload ensuring that members? needs are met. Including all appropriate parties in the individual?s Integrated Care Team, including primary care providers, therapists, and others as indicated by the member. Appropriately engaging with and refers to the resources of the Behavioral Health Care Manager and Nurse Care Manager on the Integrated Care Team to effectively meet the specialty needs of members. Facilitating collaboration and consultation with internal and external service providers including but not limited to use of specialty care managers to meet member needs in a timely and efficient manner. Responding to member requests and makes referrals to other services and resources as needed in a timely manner in accordance with program requirements. Qualifications/Requirements Community Health Worker Certification and/or Bachelor\'s Degree preferred. High School Diploma or GED required; Bilingual, Spanish and English Speaking preferred. 1-2 years of work experience in a related behavioral health, human services and/or medical field required. Demonstrates cultural and linguistic competence in dealing with members with a variety of different cultural backgrounds and linguistic needs. Demonstrates willingness and ability to serve challenging and diverse member populations and demonstrates cultural competence in dealing with members with a variety of differing cultural backgrounds. Demonstrates willingness and ability to treat challenging and diverse member populations. Demonstrates working knowledge of community resources and how to help members access them. Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management. Professional demeanor in communicating with members, colleagues, and other providers. Ability to communicate clearly and effectively with members, providers, and staff both verbally and in writing. Familiarity with Internet, email systems, and electronic health records (EHRs). Reliable and legally registered vehicle required during all working hours. Valid driver\'s license required. Has a working knowledge of CCA KPI\'s and performance metrics.
