Description:

Brightside and Mercy Medical Center members of Trinity Health of New England is looking for an experienced Behavioral Health Specialist / Social Worker to come join our team of highly specialized integrated behavioral health professionals.

Behavioral Health Specialist (BHS ), Masters Level Sis integral to population health management, including the integration of primary and behavioral health care through support of the physician and the collaborative care team. The BHS works with patients who have behavioral health conditions and focuses on social determinants that impact health outcomes. Essential functions include crisis and behavioral health interventions, connection to social and community services, and follow up to improve the patient experience, health outcomes, streamlining of services and lowering of costs.

What will you do:

  • Builds caring relationships with target population to promote positive health and social service outcomes.

  • Improves access to health and human services for target population through in-person interactions, telephone contacts, and coordination of referrals.

  • Coordinates and/or completes all assessments and goals that are strength-based, solution-oriented to build treatment plans with family and service providers.

  • Ensures implementation of the treatment plan with all providers.

  • Teaches strategies change by assessing stage of change and providing education appropriate for readiness.

  • Participates in case conferences, staffing meetings, treatment teams and other consultation meetings.

  • Demonstrates problem solving, conflict resolution, and positive communication through teamwork. Support Brightside team as needed to fulfil clients needs.

Minimum Qualifications

  • Education: Master’s degree in a social work field including, Social Work; Marriage and Family Therapy; Psychology; and/or Mental Health Clinician.

  • Licensure: Licensed or with the capability to be licensed preferred.

  • Experience: Requires one (1) to three (3) years of experience working with at risk populations. Experience with wrap-around services required.

  • Requires the ability to communicate with target population. Requires culturally Competent Interpersonal & Communication Skills

  • Must possess a comprehensive knowledge of the local community.
