Who we are: Behavioral Health Network is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities The One Care program assists adults ages 21-64 with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes. What You\'ll Do: Lead and Inspire: Oversee and mentor care teams to deliver quality care with compassion and expertise. Collaborate for Success: Work hand-in-hand with the Program Director to ensure the program runs smoothly and meets its goals. Drive Excellence: Implement administrative policies and continuously improve care through data-driven strategies. Support Growth: Develop new programs and funding streams to help the One Care program grow and thrive. Impact Lives: Directly influence the health and wellbeing of the individuals we serve by fostering integrated, person-centered care. Be a Leader: Support your team through training, supervision, and continuous feedback to ensure they succeed and thrive. A forward-thinker, embracing quality improvement and continuously striving for better results. What You Bring: An Associate\'s degree (Bachelor\'s or Master\'s preferred) in a related field. In lieu of a degree, a minimum of 5 years\' experience working in Care management or population health programs required. Community Health Worker Certification Preferred. A minimum of 2 years of supervisory experience A deep understanding of mental health, substance use, and social determinants of health. Must have Familiarity with internet, word processing and email systems. Bilingual skills (Spanish/English) are a plus. Valid Driver\'s License and reliable vehicle during all work hours required. Additional Information Schedule Days - Weekdays Schedule (Days/Time) - Monday - Friday 8am- 4:30pm # of Hours Per Week - 40
