Kennedy-Donovan Center Case Manager - Family Support Center in Sandwich, Massachusetts

About this role:

The Family Supports Program provides respite assistance to caregivers of adults with developmental disabilities. The Personal Support Staff will act as respite providers to allow caregivers to take a break from caring for a family member with a disability. The Personal Support Staff will supervise individuals in Sandwich and provide skill-building and life-skill coaching.

Schedule: 

  • Monday, Tuesday, Thursday, and Friday: 8:30 am - 4 pm

  • Wednesday: 9:30 am - 5:30 pm

    We are looking for people who:

  • Bachelor's Degree, preferred. High School/GED, required.

  • Valid Driver's License and reliable vehicle, required.

  • At least 2 years' experience working with individuals with disabilities, required.

  • Experience in community or center-based supports for individuals with developmental disabilities, preferred.

    The physical requirements for this role:

  • Frequent movement within the classroom including but not limited to kneeling, bending, sitting, lifting

  • Operation of adaptive and specialized equipment

  • Ability to physically assist individuals as determined by their needs

  • Ability to tolerate significant amounts of walking, standing, and sitting

    In this role, you'll get to:

  • Support individuals with disabilities in a group setting.

  • Promote individual choice and self-expression based upon quality service principles.

  • Implement individual and group developmental activities as assigned

  • Facilitate assessment and development of individual treatment goals and objectives by implementing and documenting individual programs

  • Ensure completion and maintenance of documentation as required by KDC and all applicable funding agencies.

  • Participate in staff, team meetings, group, and individual supervision

  • Maintain upkeep of the Family Support Center

  • Ability to communicate and document clearly 

  • Maintain agency standards of confidentiality

  • Perform other job-related responsibilities as assigned
