Eliot Community Human Services Inc Housing First Coordinator in Revere, Massachusetts

Housing First Coordinator- Emergency Family Shelter- Bilingual (Haitian Creole) Responsibilities: Screen and assess family needs within initial intake. Flag high risk factors reported or observed and immediately report to shelter leadership to formulate a plan of action to support the family. Provide milieu coverage as needed to ensure overall safety of the site and to ensure that community rules are followed. Assists guests with transportation arrangements and tangible resources to address immediate needs. Carry an assigned caseload of families and collaboratively coordinates care and supports to address assessed needs and service delivery preferences. Ensure that supports provided to families are culturally and linguistically appropriate, family centered, and trauma informed. Attend training forums as assigned. Attend weekly supervision with Site Manager or designee. Collect and input relevant data correlated to client assessment and outcomes in ETO or other assigned portal. Maintain accurate case records of initial assessment, service activities, and progress notes in electronic medical record/ETO platform within established timeframes and deadlines. Provide comprehensive service and care coordination through collateral contacts as indicated while leveraging pre and post tenancy support services within a Housing First framework. Engage families in viable pathways toward housing and tenancy sustaining resources through relevant assessments and service referrals. Assist with and participate in strategic planning for the Family Shelter Site. Build upon existing partnerships with community-based organizations to cultivate more robust support service opportunities as applicable. Serve as a member of internal/external multi-disciplinary teams and case conferencing forums as assigned. Develop and maintain professional working relationships with residents, staff, co-workers and supervisor. Work effectively with diverse populations and cultural groups. Complete and track USCIS immigration profiles and status, vocational assessments through MassHire, and relevant contact/reporting information per EOHLC guidelines within ETO system as needed/assigned. Incorporates individual/family strengths into assessments. Support families with maximizing state and federal entitlements such as MassHealth, EADC/DTA benefits (SNAP/Food Stamps/Cash Assistance) as well as other social, recreational, and/or educational opportunities that foster enrichment and growth. Collaborate closely with housing navigation team to enroll in HomeBase program, complete CHAMP application, unit identification in geographical area of preference, application process for unit, viewing and obtaining vital documentation as needed and relevant. Recognize and respond to signs and symptoms of changes in client behavior, physical health or related medical issues, or other related areas of concern and seek supervisory support/consultation as needed. Qualifications (one of the following): Bachelor?s degree in human services or related field required. For candidates with lived experience with substance use disorder who have 2 years sustained sobriety. High school diploma/GED and at least one year of paid or volunteer experience working with adults with mental illness and/or substance abuse. Must have a strong commitment to substance use recovery and housing first philosophy. Must have skills to establish supportive relationships with persons with mental illnesses/substance use and respect for their preferences for treatment. Ability to provide flexible services that are community based and focused on team work.
