Maxim Healthcare Staffing RN Case Manager in Plymouth, Massachusetts

Maxim Healthcare Staffing is seeking a Nurse Case Manager who is responsible for coordinating the continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery and compliance with medical regime.


  • Performs andcoordinates the initialassessments and ongoing reassessments of the patient's status

  • Documentspatient case information withina database system

  • Performschart review/audits monthlyor as needed

  • Participatesin monthly case conferences by providing informationpertinent to patient’s needs/goals

  • Partners with the Program Director in development and review of the patient’s individualized coordination of care plan

  • Ensures that the patients’ medical needsareaddressed; consults withthepatients’ physicians as needed, coordinatingplans of treatment, andadvocating for the patient when necessary

  • Promotesunderstanding of the medical factors affecting the targeted population

  • Identifiesand assists patient(s) in accessingentitlements, resources, information, andreferrals for psychosocial needs

  • Participatesin Quality Assurance and UtilizationReview activities, as directed

  • Empowerspatients in decision-makingfor care planning

  • Maintainsaccurate and timely patient information,which is readily accessible for review and meetsallrequirements; assistsin data collectionfor reporting/funding sources

  • Fostersintra-facility andinter-facility working relationships to help accomplishgoals; acts as a liaison between primary care providers, specialist, and/or patient

  • Advocates on behalf of patientregarding accessibility of services

  • FollowsState/Country mandated guidelines for the nurse casemanagement programs

  • Participatesin outreach activities to theentire target population, as directed

  • Recommends program/servicechanges to meet gaps inservice in the community

  • Performs other dutiesas assigned/necessary


  • Current RN license

  • At least one year of Case Management experience preferred

  • Current BLS cardpercontract requirements

  • Current TB or Chest x-ray(if applicable)

  • Ability to effectively elicit/provideinformation to and from appropriate individuals(including, butnot limitedto,supervisors, co-workers, clients)via strong communication skills;proficiency intheEnglish language may be required

  • Computerproficiency required

  • Must be at least 18 years of age
