State of Massachusetts Case Manager - Child & Youth Division in Plymouth 38 Industrial Park Rd, Massachusetts
The Child Youth and Family Services operated by the Department of Mental Health (DMH) at the Plymouth Site Office is seeking a dynamic candidate who will provide Case Management services for eligible youth under age 22, with majority of caseload below age 18. The successful candidate will have knowledge and desire to work with youth with mental health need and their families to promote recovery and connection to community-based services. As a Southeast Area CYF staff, the Case Manager will provide assessment, outreach, support, and service coordination to DMH authorized individuals by promoting DMH’s philosophy of recovery, trauma informed care, person, youth and family-centered choices _Please Note:_ The Work Schedule is* Monday through Friday 8:30am to 5:00pm (40 hours/week) with Sunday and Saturdays off. * * *_Duties and Responsibilities:__ (these duties and responsibilities are not all inclusive)_ * Provide case management services for eligible youth under age 22, with majority of caseload below age 18. Engage families/guardians in the process to align with CYF Family Collaboration Principles. * Provide creative engagement strategies including regularly scheduled face-to-face contact in accordance with the needs of the person served, family and other community providers to engage and sustain involvement in attaining the individual’s goals and maintaining community tenure. * Work with a team of internal and external providers and family to ensure integrated planning and service provision. * Prepares and maintains documentation according to DMH regulations and Electronic Health Record (EHR) business rules including progress notes and other required documentation to ensure accountability and effective and efficient service delivery. * Provide regularly scheduled visits occur in the client’s home or usual place of residence, as appropriate, and monitors safety of the environment. * Coordinates/interacts with multi-service/system team (family, team, school, other state and community systems), organizes, schedules, facilitates (through regularly scheduled meetings/telephone contacts), coordinates and monitors service implementation * Plan, convene and chair meetings of client’s care planning team to discuss Individual Service Plan effectiveness and modifications, coordinate service delivery and other assistance. _Required Qualifications:_ * Knowledge of psychosocial problems, types and symptoms of those with mental and emotional disorders. * Knowledge of the principles, practices and techniques of counseling, interviewing, crisis intervention, motivation and reinforcement. * Knowledge of the types of programs and services available to the mentally ill population. * Ability to understand, explain and apply the laws, rules, regulations, policies, procedures, specifications, standards and guidelines governing agency and Site activities. * Ability to make case assessments by examining records and documents and through questioning and observing clients. * Ability to communicate effectively orally and in writing, giving clear accurate instructions. * Ability to make decisions, act quickly and maintain a calm manner in stressful or emergency situations. _Special Requirements:_ Travel is required - This position directly manages cases in the DMH Quincy Site catchment areas and services in which will be primary travel locations; however, frequent travel is required throughout the DMH Southeast Area and occasional required statewide travel to various acute inpatient hospitals or other treatment settings. Current and valid Massachusetts Class D (or equivalent from another state) Motor Vehicle Operators license required. * *_Preferred Qualifications:_* * * 3 years experience in child/adolescent behavioral health services. * Multilingual fluency __ _About the DMH Child Youth and Family Program:_ Child, Youth and Family (CYF) Case Manager Supervisor is responsible for ensuring direct case supervision and consultation to CYF Case Manager(s) in order to ensure compliance with established standards of care for Case Management for the Site. The Department of Mental Health provides Child, Youth, and Family Services to eligible individuals and families who meet the clinical criteria for service authorization. Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines.
