Competitive pay based on experience: Admissions Assistant \$20.00 - \$23.00 per hour At Hillcrest Commons Nursing & Rehabilitation Center, we have been caring for area families since 1995, providing top-quality skilled nursing care for short-term rehabilitation, long-term care and specialized memory care. Essential Job Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. - Schedule and conduct tours for prospective families and applicants. Share pertinent information regarding this facility, as well as any other Integritus Healthcare facilities or services that are appropriate. Obtain any additional information needed from the family for the admission process. - Act as the intermediary between the liaisons and collect referral (clinical and financial) information. Obtain clinical and financial approval through appropriate channels with facility leadership within 15-30 minutes and let the liaison know the status. - Assure the appropriate facility managers have the appropriate information to coordinate resident care for new admissions. Coordinate with rehab, social work, nursing, dietary and other departments as needed. - Obtain clinical approval from nursing management prior to admission of the resident. - Assure payor source is clear prior to admission. - Complete admission paperwork with the resident and/or appropriate family member. - Assure OBRA and PASAAR regulations are met prior to admission. - Notify all managers of new resident admissions. - Assist with coordinating customer satisfaction survey, welcome and discharge transition surveys. - Act as a champion for customer service in the facility. - Support the maximization of census. - Track referrals in database. Give administrator referral information as needed. - Attend mandatory in-services held at the facility. - Understand and adhere to all safety, infection control, sanitation, accident, fire, disaster and personnel policies and procedures of the facility. Promote a culture of safety. - Discuss any problems, needs or concerns that arise with the appropriate manager and/or the administrator. - Understand and uphold the Residents\' Bill of Rights and hold all resident information confidential. - Represent this facility and the organization in a professional manner. Maintain a professional appearance and mannerisms according to facility standards. - Maintain an acceptable level of attendance in accordance with facility policy. - Display a polite and courteous manner to all fellow employees, residents, and visitors.
