Signature Healthcare Patient Navigator/Medical Assistant in North Easton, Massachusetts

The Patient Navigator/Medical Assistant will assume responsibilities for the care coordination for patients. Works collaboratively with internal and external entities in order to remove barriers to care and ensure smooth care transitions for patients/families. The Patient Navigator/Medical Assistant works with patients to address their needs by serving as the link/liaison between the patient and the rest of the care team. The Patient Navigator/Medical Assistant will identify and help to close care gaps, provide support and education to the patient and family/caregivers, and keep the patient fully involved in their healthcare. Experience in community services and medical office preferred. High School diploma or equivalent. Graduate of an approved Medical Assistant certification program. One-two years of successful working experience as a Medical Assistant in a medical practice setting preferred. Current American Heart Association BLS for Healthcare Providers completion. Medical Assistant Certification through one of the following national organizations:
