Child & Family Services Therapeutic Group Facilitator (BA) in New Bedford, Massachusetts

Job description The Therapeutic Group Facilitator will ensure open access to services and community resources to clients by facilitating an array of psychoeducation, support, and community workshop groups in the clinic and community. The Therapeutic Group Facilitator will also identify and link clients and families with the appropriate level of care to best support their needs. \$1 per hour Language Differential! (eligible languages include: Cape Verdean Creole, Haitian Creole, Portuguese, Spanish or ASL) Qualifications: Bachelor?s degree in social work or related field from an accredited educational institution required and one (1) year of experience working with the target population; or a completed master?s degree in clinical social work, Clinical Mental Health Counseling, Clinical Psychology, or other clinical licensure eligible degree program is required Candidates with language capacity strongly encouraged to apply Responsibilities: The following responsibilities are not meant to be all inclusive and may be adjusted to meet the agency?s needs. Provide a safe, welcoming, and clean environment for our clients and families Coordinate and participate in the facilitation of psycho-educational, support or community workshop groups with children, adolescents, and adults which may take place in a clinic, school or community setting. Facilitate minimum of 8-12 groups per week Complete necessary intake paperwork including pre-screenings, releases and consent forms with the clients and family?s permission. Provide psycho-education to clients and families regarding their rights and responsibilities Collect and gather pertinent information from clients and families to better understand their symptoms, behaviors, history, identified strengths and needs, overall functioning, treatment goals and identify preferences to determine appropriate level of care and services Conduct case management meetings regularly with clients and or parents/caregivers and other stakeholders to discuss progress toward client-centered goals and to modify individual service plans as needed Work in collaboration with team members, clients and families to implement person-centered and family-centered, recovery-oriented, evidence-based and trauma-informed care Refer clients needing additional services to the appropriate resources Provide crisis management services and coordinate with Emergency Services as appropriate Utilize evidence-based interventions, including motivational interviewing skills to empower clients and families with improving their overall health and wellness Engage in effective coordination of care across health and behavioral health services to facilitate wellness and recovery of the whole person Attend meetings and participate within the multi-disciplinary teams as needed Maintain a flexible schedule that is conducive to providing services in an open access model that will include in-person and telehealth services Maintain record-keeping as required by CFS, funding sources, and accreditation authorities Adhere to SAMHSA, the CCBHC, and CFS expectations, policies and procedures Participate in all supervision and training activities as required by CFS, funding sources, and accreditation authorities Keep EHR calendar up to date to support centralized scheduling If licensed, complete continued education to maintain clinical professional licensure and ensure that licensure remains active
