Qualifications
• Graduate of an accredited College of Pharmacy
• Current MA Pharmacist License
• Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification (if applicable) or within 6 months of start date
• PGY 1 + 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area; BPS or obtained within 1 year of hire
Responsibilities
• The position’s designated specialty area will be Antimicrobial Stewardship and Critical Care
• Eighty (80) percent of the employee’s time will be spent working in the designated specialty areas of Critical Care and Antimicrobial Stewardship
• Through various assignments within the department, provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients within their specialty
• All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory
• Participates in the didactic and experiential training of pharmacy students and residents
• Performs presentations for the organization
• Maintain a portfolio that outlines information about yearly requirements
• Annually, specialists will be expected to complete MUE, CE or In-Service
• Continued service-line expansions, relevant order set reviews, policy and/or procedure development/modification and formulary monographs may be required when needed
• Responsible for leading local meetings at the direction of the Director of Pharmacy
