Eliot Community Human Services Inc PACT General Case Manager in Malden, Massachusetts
Responsibilities: Provide case management services as part of a multidisciplinary team Develop treatment plans with consumers. Act as a primary case manager to consumers in accordance with Eliot?s policies and PACT standards. Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner. Assess client\'s functions, strengths and needs. Develop therapeutic connections with each client and provide clinical interventions. Provide on-call on a rotating basis and rotating evening shifts. Provide problem-solving supports and interventions to clients Schedule: Monday - Friday 8am to 4pm with standing 12pm-8pm shift. Includes rotating weekends and on-call. Education Required Bachelors or better in Psychology or related field. Experience Required 2 years: Experience with adults that have persistent mental illness Licenses & Certifications Required MA Driver\'s License Requisition Number: PACTG008764
