Actalent Case Manager in Malden, Massachusetts

Job Description

We are seeking a dedicated and compassionate Case Manager to join our team. In this role, you will be responsible for communication and relationship building with clients and all associated partners (schools, funding partners, therapists, agencies, foster homes, etc.). You will set limits for youth, develop interventions, and set goals towards individual successes in conjunction with clients’ guardians, therapists, and other providers. Participation in on-call services as needed is also required.

Responsibilities

  • Build and maintain relationships with clients and collateral partners (schools, funding partners, therapists, agencies, foster homes, etc.)

  • Set limits for youth and develop interventions

  • Collaborate with clients' guardians, therapists, and other providers to set individual success goals

  • Participate in on-call services as needed

Qualifications

  • Bachelor’s Degree

  • LSW or LSWA license or license-eligible

  • Experience in case management, care management, and foster care (Preferred but not required)

  • Valid Drivers License and reliable vehicle for travel

Pay and Benefits

The pay range for this position is $52000.00 - $52000.00/yr.

Available benefits for this opportunity include 401k Plan with match, Paid Time Off, Sick Time, Personal Days, 12 Holidays, Mileage Reimbursement.

Workplace Type

This is a fully onsite position in Malden,MA.
