    Position Overview : The Supportive Housing Coordinator (SHC) ensures that residents receive the supportive services needed to maintain their independence, and integrates social and recreational activities into the housing facility.

    Position Responsibilities : Identify residents with unmet needs and assist them with obtaining community benefits, resources or services. Provide outreach, advocacy, and crisis intervention accordingly. Support tenants in making day-to-day adjustments to supportive housing living. Assess individual resident's needs. Plan, coordinate, and monitor the delivery of services to residents in the building as well as identified community consumers. Handle on-site emergency issues with consumers. Provide Case Management services to all State Home Care clients and private pay clients. Perform the responsibilities of a Home Care case manager including maintaining site-specific and community caseload, intake for building residents, assessment and interviewing, home visits, service planning, case conferences, service referrals and coordination, advocacy, and documentation according to the standards and regulations of EOEA.

    Qualifications

  • Experience in the Human Services field or Bachelor's degree desired

  • Bilingual Spanish required

  • Knowledge of community resources

  • Possess the knowledge and skills necessary to work successfully as a member of an interdisciplinary team to provide or arrange for community long-term care

  • Ability to make proper judgments, demonstrated leadership qualities, good written and verbal skills, and ability to maintain effective interpersonal relationships with staff members, consumers and partners

  • Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook

  • Ability to process information accurately

  • Ability to work both independently and as part of a team

  • Professional demeanor and ability to handle confidential materials with discretion

  • Must have reliable transportation and valid driver's license

    Hours:

    Full Time - 40 hours Monday through Friday
