Position Overview: The Long-Term Service Support Coordinator will be working with a multitude of diverse partnering agencies across both health and social service disciplines. This is dedicated to promoting consumer choice and self-management as well as greater community inclusion, access, integration, mobility, and robust livability.
Position Responsibilities: Responsible for providing comprehensive and enhanced person-driven supports to enrollees, ages 3 to 64, that are participating in the Commonwealth's Long-Term Services and Supports Community Partnership program. You will also facilitate enrollee use of assistive technology, sustain enrollee engagement and encourage problem solving, manage date and supporting outcomes evaluation.
Qualifications:
•Bachelor's degree in Human Services or a related field or
•An Associate's degree and at least one year professional experience in the field; Community Health Worker (CHW) or similar certification
• Bilingual candidates will be given priority consideration
•Previous work experience working with persons with disabilities a plus (children and adults)
•Detail oriented
•The ability to appropriately handle confidential and personal information
•Proficient computer skills
•Strong writing and communication skills
•Reliable transportation and a valid driver license to make visits throughout the GLSS service area
Hours 40 hours per week, Monday through Friday
