Description
Eliot's behavioral health program is seeking an energetic, empathic, Clinical Leader to direct an interdisciplinary team in the coordination of medical, behavioral health and Social Determinants of Health (SDOH) needs. The ideal candidate will have a strong commitment to mental health recovery, substance abuse disorders and the coordination of integrated care.
Eliot's One Care Health Home program is an innovative Care Management approach for members with complex medical, behavioral health, and social needs who are dual eligible, having both Masshealth and Medicare benefits.
Currently seeking a Clinical Leader for the Health Home Initiative within Eliot's Behavioral Health Services.
Qualifications: Clinical Licensure required. Must have at least four years of experience working with people with severe and persistent mental illnesses, substance abuse disorders, or those with complex medical conditions. Must have at least two years supervisory experience. May require additional or specialized training with specialized populations and working knowledge of Evidence Based Practices. Must have a strong commitment to recovery and care coordination. Must have skills to support members to define and achieve their goals, and build/maintain independence. emergency contact.
Responsibilities:
Oversee the clinical operations of the Health Home Outreach Care Team.
Provide supervision and clinical guidance on Annual MDS Comprehensive Assessment, Person-Centered Care Plan (PCCP) and the Member Engagement process. Provide rotating 24/7 On-call coverage back up. Provide quality assurance oversight of Member’s centralized record. Clinical oversight over multiple Health Home Teams.
Ensure coordination of services, assessments and treatment planning to empower enrollees to exercise autonomy in all aspects of their life.
Provide overall clinical supervision and oversight of care coordination and service delivery of assigned Health Home Team(s).
Provide clinical guidance and support in developing interventions, engagement, and care management.
Teach and model clinical interventions, strength-based strategies and ensure services are culturally competent.
Complete all required assessments, complete crisis plans, and develop strategies with the Health Home Team to coordinate aspects of enrollee care in conjunction with completed assessments and crisis plans.
In collaboration with the enrollee, Health Home Team, and the enrollee’s PCP, ensure completion and review of enrollees’ assessments.
In collaboration with the enrollee, Health Home Team, other Care Team members, and the enrollee’s PCP ensure completion and review of enrollees’ Person-Centered Care Plans (PCCP).
Schedule: Monday-Friday, 9am-5pm
Location: Hybrid. Oversee staff that work in Danvers, Lynn, Concord, Malden, and Saugus
Salary: 80k annually
