Job Description:

Hospice RN Case Manager

Monday through Friday/8:00 am to 5:00 pm/Lowell-Tewksbury Coverage area

Key Responsibilities

In this role as a Hospice RN Case Manager, you will:

  • Assess and address the physical, psychosocial, and environmental needs of hospice patients in their homes.

  • Conduct thorough initial and ongoing evaluations of patients’ nursing needs at each visit.

  • Provide regular health updates to clinical supervisors, physicians, and the care team.

  • Develop and implement personalized care plans, guiding and supporting patients and their families.

  • Maintain quality of care by following individualized plans focused on achieving positive health outcomes.

  • Identify and respond to any changes in a patient’s health, taking timely action as necessary.

  • Ensure care aligns with each patient’s Plan of Care, following physician orders and clinical best practices.

  • Take on additional duties or projects as assigned.

What You’ll Bring:

  • Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Bachelor’s).

  • Current State RN license.

  • Minimum of 1 year of clinical experience, with hospice or home health experience preferred.

  • Strong nursing skills and familiarity with accepted nursing standards.

  • Ability to manage multiple priorities, deadlines, and documentation requirements.

  • Reliable transportation for home visits.

  • Highly preferred to live in or near the service area
