Case Management Resources Specialist

Works collaboratively with the Complex Care Nurse Managers, Social Workers, and Utilization Review Nurses, in coordinating and providing care that is safe, timely, effective, efficient, equitable and client-centered. Assuming all administrative responsibilities within the admission and discharge planning, Perform other duties as needed and assigned by your supervisor.

JOB RESPONSIBILITIES:

  • Perform the full range of duties, working independently, applying well-developed subject knowledge, and exercising judgment and initiative .

  • Provides administrative, technical, and direct support to Complex Care Nurse Managers; complete admission patient interviews and care needs assessment, collect, enter, and maintain information in the client files and EMR; help case managers coordinate care across the continuum.

  • Queries patients to past Visiting Nurse Agency (VNA) and Pharmacy preferences

  • Verifies the presence of Health Care Proxy (HCP), if none on file completes one with the patient prior to admission and transfer to the unit.

  • Ensures each patient admitted receives notice of admission, admission packet, and is informed of their rights as a patient.

  • Perform limited intake to determine initial and ongoing financial needs and screen for unmet service needs; contact community resources as an advocate for clients.

  • Ensures Important Message (IM) notices for Medicare patients have been distributed and completes all required IM documentation

    MINIMUM EDUCATION AND EXPERIENCE:

    REQUIRED:

  • High School Diploma, GED or equivalent

  • Knowledge and understanding of medical terminology

  • Strong organizational skills

  • Flexibility and accountability

  • Demonstrated verbal, written skills

  • Interpersonal skills required

  • Critical thinking and knowledge of regulatory standards and reimbursement requirements

  • Computer skills required

    PREFERRED:

  • Bachelor’s Degree
