State of Massachusetts Community Mental Health Worker III in Hyannis, Massachusetts

The Department of Mental Health's Cape & Islands Community Mental Health Center is seeking a dynamic candidate for a Mental Health Worker (MHW) III position within the Adult Community Clinical Services (ACCS) Integrated Team. The successful candidate will be a part of a multi-disciplinary team that provides comprehensive services to individuals living in the community. The MHW III will be responsible for providing trauma-informed, person-centered community-based interventions that target skill development to an assigned caseload of community clients. Will provide direct supervision of MHW staff and assist in the handling of Individualized Action Plans (IAP). Participates in the development of Community Service Plan (CSP) with assigned caseload. Completes required documentation and communicates critical and pertinent information to team. _Please note_: This position is 40 hours per week. The selected candidate will work Sunday through Thursday 8:30am - 5:00pm. Days off are Fridays and Saturdays. * *Employees in this position will be required to work mandatory overtime. _Duties and Responsibilities (these duties are a general summary and not all inclusive):_ * Serve as a member on an ACCS Integrated team by attending regularly scheduled meetings, coming prepared to present relevant psychosocial history, and provide updates on person served's progress toward goals and any barriers to progress. * Collaborates with Residential Supervisor & MHW II's to provide rehabilitative interventions to GLE clients and assist with development of pre-independent living skills. * Assess the needs of assigned persons on a regular basis by reviewing records, providing supportive counseling and maintaining regular contact with persons served and families via outreach in their home or other community environments. * Responsible for reading and communicating all pertinent information and attending daily rounds and staff meetings and informing supervisor or designee of any critical information. * Responsible for transporting or arranging transportation for persons served that need that level of service. * Involve persons served and all identified natural supports and collateral providers in the process of treatment planning and ensure that plans are agreed upon by obtaining person served or LAR signature. * Responsible for maintaining person served records in a timely manner by documenting in EMR and paper record and completing all other related forms to ensure compliance with DMH regulations and Rehab Option requirements. * Knowledge of all medications used within the program, including their indications, risks and side effects, and the ability to teach person served about their medications and how to take them appropriately by utilizing MAP related material and training when needed. * Provides culturally and linguistically competent services that are also sensitive and responsive to religious practice, sexual orientation, gender differences, physical disabilities, as well as services that are age and developmentally appropriate, including young adults and elders. * Responsible for ongoing communication with other treatment providers to assure coordination of treatment planning and for sharing of pertinent information. _Preferred Qualifications_: * Previous experience working with people with chronic mental illness and/or dual diagnosis. _Special Requirements:_ Please note in accordance with an agreement between the commonwealth of Massachusetts, Department of Mental Health and AFSCME/Council 93, employees assigned to a work unit where the Medication Administration program (MAP) is used are required to pass MAP certification with 6 months of entering the position or 3 examination opportunities, whichever comes first.
