GREENFIELD - Re-entry Case Worker Do you want a career that empowers individuals and strengthens your community? CHCFC is seeking a passionate Reentry Caseworker to join our dedicated team. In this role, you will play a vital role in supporting individuals returning from incarceration as they reintegrate into society. You will provide guidance, resources, and advocacy to help them achieve their goals in housing, employment, and overall well-being. Key Responsibilities: Conduct intake assessments to evaluate clients\' needs, strengths, and barriers to successful reentry. Develop individualized reentry plans in collaboration with clients, focusing on areas such as employment, housing, education, and healthcare. Provide ongoing case management and support to clients, including referrals to community resources and services. Advocate for clients\' needs and rights within the criminal justice system and in the community. Facilitate group workshops and support groups on topics related to reentry, life skills, and personal development. Maintain accurate and up-to-date documentation of client interactions and progress towards goals. Collaborate with other agencies, organizations, and stakeholders to coordinate services and support for clients. Participate in trainings and professional development opportunities to enhance knowledge and skills in reentry services. Qualifications: Bachelor\'s degree in social work, psychology, criminal justice, or a related field. Previous experience working with individuals involved in the criminal justice system or in reentry services. Strong understanding of the challenges faced by individuals reentering society from incarceration. Excellent interpersonal and communication skills, with the ability to establish rapport and build trust with clients. Empathy, patience, and nonjudgmental attitude towards clients. Ability to work independently and as part of a team, with strong organizational and time management skills. Proficiency in Microsoft Office and case management software. Additional Information: This is a full-time position based in Greenfield, MA. Some evening and weekend hours may be required. Competitive salary and benefits package offered.
