LifePath, Inc Bi-Lingual Resource Consultant in Greenfield, Massachusetts

GREENFIELD - Bi-Lingual Resource Consultant - Spanish & English LifePath is a mission-driven, collaborative, and community-focused human services agency seeking to hire a person who is bi-lingual in Spanish and English to support our program providing information and referral, follow-up and advocacy to callers and who will process intake information for internal programs operated by LifePath. The Resource Consultant serves as the public\'s entry point to LifePath\'s services. Join our caring and compassionate team! In this full-time (35 hr/wk), hybrid role based in Greenfield, pay is \$22.90 per hour, with excellent benefits including medical, dental, vision, life insurance, 401k, HRA, FSA, EAP, and generous paid time off! QUALIFICATIONS: Bachelor\'s Degree in a Human Service related field preferred with years\' experience working with older adults and caregivers The position requires: English and Spanish fluency Ability to work independently and within a team Excellent customer service skills Strong verbal and written communication Knowledge of community resources and ability to research options Sensitivity to work with diverse populations in their homes, virtually and telephonically. Comfortable facilitating outreach events and presentations for individuals and groups. Ability to work within databases, use Google and Microsoft Office Suite, virtual platforms, and other technology. Ability to travel/drive throughout Franklin County and North Quabbin region in varied weather. ESSENTIAL DUTIES & RESPONSIBILITIES: Perform professional interviews over the phone to determine needs of callers to make appropriate referrals. Complete referrals and intake assessments as required. Maintain up-to-date and comprehensive knowledge of available resources for older adults, caregivers, and people living with disabilities. Maintain current understanding of relevant regulations and application procedures of entitlement benefits. Maintain up-to-date and comprehensive information of available resources for elders, caregivers and individuals with disabilities. Conduct community presentations and outreach activities through public speaking engagements or related activities. Provide follow-up communication with consumers as needed to ensure outcomes of previous contact and offer additional assistance in locating other services as necessary. Maintain accurate and up-to-date documentation according to program and agency requirements of Information and Referral (I&R) calls, follow-up and advocacy work. Complete daily call log in database on all calls. Participate in regular agency meetings and team meetings and attend in-service trainings.
