Berkshire Health Systems Social Worker III in Great Barrington, Massachusetts
DEFINITION/PRIMARY FUNCTIONS
The medical care management social worker provides assessment and intervention to patients and families undergoing acute medical care and treatment. This intervention may include any or all of the following; discharge planning, case management, counseling, assistance with complex financial issues, legal issues and regulatory reporting.
POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
Experience:
One (1) year case work experience in a hospital, social service agency or counseling center required.
Education and Training:
Master's degree in social work required
License, Certification & Registration:
LCSW - Licensed Clinical Social Worker required.
LICSW preferred
Other Requirements:
Must possess sight/hearing senses or use protheses that will enable these senses to function adequately so the requirements of the postion can be fully met.
Excellent interpersonal and communication skills required.
Ability to work independently required.
Proven crisis intervention skills.
Proven solution oriented and brief therapy skills.
Knowledge of BMC/BMHC resources and community resources.
Knowledge of regulatory reporting requirements.
Ability to maintain confidentialty.
Proven collaboration skills with other disciplines.
Computer experience preferred
