This position is part of the High Risk Intervention Team (HRIT) providing psychiatric services and connection in the community for our clients enrolled with the High Risk Intervention Team or our Moms Do Care Program.

As a member of the High Risk Intervention Team, the NP/PA will work directly with patients, families, caregivers and providers. The NP/PA will assess patients to identify signs of behavioral health disorders, including substance use disorder, and will work collaboratively with emergency department, inpatient staff, HRIT staff and Moms Do Care staff. The NP/PA will educate patients about medication options and if needed will administer first dose of medication.

The NP/PA will also provide support to the High Risk Intervention Team including the Moms Do Care program. Additionally, the NP/PA will consult with medical practices as needed. The NP/PA will work collaboratively with outpatient clinic providers to ensure that patients initiated on medications have follow up, longer term services. The NP/PA will report to the Program Director of the High Risk Intervention Team. The NP/PA will be required to obtain DEA X waiver certification to be able to prescribe MAT under the guidance of supervising MD.
