Heywood Hospital MAHEC Grant Coordinator, MAHEC, 40 Hours, Day Shift in Gardner, Massachusetts

Job Description The MAHEC coordinator will work with the Youth Tele BH Manager to implement this a program. Thisprogram will purchase carts for school districts which will allow NPs to tele into school nurses offices and perform tele exams on students for acute/non-acute appointments. The coordinator will work with all parties affiliated with the grant, develop a school nurse training schedule, purchase equipment and implement the program in participating school districts. The goal is to develop a whole child approach to health care and address both Behavioral Health and Physical Health in our schools Job Requirements Minimum Education * High School Diploma or equivalent required. * Associate's degree preferred. Years of experience can substitute for education requirement. Minimum Work Experience * Experience working in the Tele Health field, working with established teams and school-based programming. Required Skills * Possess a full working knowledge grant implementation and identification of key performance indicators, meeting grant timelines and development/implementation of training sessions Functional Demands * Physical Requirements: Exerts up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Frequently reaches (extending hands and arms in any direction), and handles (seizing, holding, grasping, turning, or working with hands) *Organizational Expectations Behavioral Attributes: Job performance reflects compliance with Heywood’s performance standards; telephone etiquette, providing directions and first impressions Essential Functions * Becomes familiar with the new grant and develop a timeline for implementation, present the timeline to Program manager for approval * Connects with grant partners and identify two schools who will initially implement the program. Expands to other districts post pilot initiative * Communicates with school partners, Northeastern University and the Program Manager to identify implementation processes, trainings and equipment needed to complete the pilot * Keep stakeholders informed of project progress and expansion and convene regular meetings with internal staff and partners * Represents the program at various local, regional, statewide, and coalition meetings and collaborates with Program Manager to present at speaking engagements and trainings * Develops a data reporting program for this grant, collect, manage and evaluate data generated from the MaHec program * Prioritizes project goals, objective and performance standard for all sub-projects, ensures their consistency with desired outcomes * Implements program activities in coordination with program staff * Organizes, directs and evaluates delivery of program services * Provides technical support and guidance to MAHEC partners as needed * Recommends improvements to increase program operational efficiency and effectiveness * Trains staff on EMR when needed and audits EMR submissions for Tele Team BH/Medical * Attend or participate in relevant trainings *Statement of Other Duties * This document describes the major duties and responsibilities for this job, and is not intended to be a complete list of all tasks and functions.
