The Arc of Opportunity Program Director of Home Based Supports in Fitchburg, Massachusetts

Schedule: Monday-Friday 8am-4pm (40 hr/week)

Summary:

The Program Director will lead the Adult Family Care (AFC) & Shared Homes team, which provides assistance to caregivers and individuals with disabilities to live as independently as possible in the community. Ensuring high quality of services and implementing policies and procedures in alignment with the organizations strategic direction. Will be mission- focused, and lead in a creative, professional and team-oriented way.

Job Responsibilities:

  • Responsible for the oversight of Adult Family Care (AFC),and Shared Homes (DDS and MFP) including the assessments of individuals for potential placement, screening of caregivers, transitional placement compliance, and evaluation of placement outcomes.

  • Supervise Case Managers and other assigned team members.

  • Work collaboratively with the Division Director of Nursing regarding Clinical Coordination of AFC nursing services.

  • Provide status updates of caseloads and staff progress to Division Director including monthly audits on site visit checklists.

  • Implements and monitors established budgets, oversee operational expenses.

  • Oversee all program participant, caregiver, and personnel records including information kept in HCSIS, Icentrix, AGD, Relias and ADP to ensure that all documentation complies with agency, state and federal guidelines

  • Ensure timely and accurate coding for billing purposes and required documentation for individuals receiving Rep Payee services.

  • Collaborate with the Division Director to create and execute program goals.

  • Participate in a 24 hour/365 day emergency response line.

  • Other duties as assigned.

Qualifications:

  • A valid driver’s license, acceptable driving record, CORI and Federal Fingerprinting are required at all times during employment with The Arc of Opportunity.

  • Bachelor’s degree required in relevant field and a minimum of five years’ experience providing supports to the individuals served in AFC and/or Shared Homes.

  • Working knowledge of AFC and Shared Living standards as well as familiarity with funding agencies in the region and other referral/outreach locations.

  • Functional and situational reasoning ability to gather and apply factual information, exercise sound judgment, problem solve and apply principles of influence systems such as motivation, team building, accountability, coaching/counseling of staff and leadership within the scope of the position.

  • Cross-functional communication skills, which convey clear direction, desired results and vision to direct reports, individuals/families served, government agencies, community partners, funding sources, peers and senior management.

  • Ability to meet the physical requirements of the job.
