What is the Adult Foster Care (AFC) program?

The AFC program is a Medicaid-funded initiative that enables individuals with disabilities to live with caring providers in a safe and nurturing environment. Whether it is a family member or a non-family member, Care Providers play a vital role in ensuring the well-being of program participants. The AFC program offers comprehensive case management, nursing oversight, and a tax-free stipend.

What does a Person-Centered Plan Manager do?

As a Person-Centered Plan Manager, you will be responsible for providing dedicated support to Care Providers in fulfilling their responsibilities. Your key duties will include:

  • . Conducting monthly home visits for program members and their Care Providers, ensuring their well-being and compliance with regulations.

  • Completing all necessary documentation accurately and promptly to maintain thorough records of each visit.

  • Maintaining open lines of communication with assigned staff and coordinating with other service providers, such as day programs and work programs, to ensure comprehensive care for program participants.

  • Making appropriate referrals to external resources when additional support is needed.

  • Monitoring the home setting according to regulatory guidelines to ensure the safety and comfort of individuals with disabilities.

    Qualifications and Skills:

  • Must pass background checks and pre-employment screenings including TB test and CORI.

  • Bachelor's degree in social work, human services, or a related field (or equivalent experience).

  • Basic computer skills using Microsoft Office, email communication, internet, and documentation entry.

  • Strong interpersonal and communication skills to effectively collaborate with program participants, their Care Providers, and a multidisciplinary team.

  • Ability to prioritize tasks, work independently and within a team, and meet deadlines.

  • Compassionate, resourceful, and eager to learn.

  • Valid driver's license and reliable personal vehicles meeting state requirements.
