State of Massachusetts Registered Nurse IV in Fall River, Massachusetts

The Dr. John C. Corrigan Mental Health center is seeking dedicated and compassionate individuals for the position of Registered Nurse IV who desire a rewarding career making a difference in the lives of individuals. Our ideal candidate for this position will work with and assist other professional staff in providing da-to-day direct care to individuals on a 16-bed secured inpatient unit with serious and persistent mental illness, including monitoring patient's behaviors, providing direct nursing care and patient teaching in regard to health and wellness, medication and illness management. The selected candidate will possess strong interpersonal skills to encourage and motivate individuals in their recovery to work towards treatment goals. _Please Note:_This position is 40 hours per week. The selected candidate will work the second shift, 2:45 p.m. to 11:15 p.m. The days off will be Week I - Tuesday/Saturday and Week II - Sunday/Thursday. Employees in this position will be required to perform mandatory overtime. _Duties and Responsibilities (these duties and responsibilities are not all inclusive):_ * Perform Nursing/rehabilitative assessments of current or prospective clients by conducting face-to-face interviews, reviewing records and consulting with providers to determine the need for services and plan for effectively dealing with client’s needs. * Complete written initial nursing assessments, including HIV, pain, trauma, risk, health status and wellness assessments for Corrigan Inpatients. * Admits and discharges patients; provides and coordinates health teaching to maintain optimum level of health and function. * Directs the nursing activities on assigned shift on the Corrigan Inpatient Unit. * Evaluates nursing activities by reviewing patient charts, observing nursing care and visiting patients to ensure that nursing care is carried out as directed and treatment is administered in accordance with physician’s instructions. * Coordinate the implementation of clients’ individual treatment service plans as determined by an interdisciplinary team, by conferring with team members and by directing and teaching line staff to implement interventions designed to assist patient in meeting goals. * Authorize overtime for shift personnel and transportation of patients to other health cate facilities for regularly scheduled appointments and in emergency situations. * Monitor and implement policies and procedures of the Corrigan Inpatient Unit to meet Joint Commission and Centers for Medicare and Medicaid Services regulations. * Ensures compliance with established standards for quality of patient documentation systems, which include patient records, patient care plans, restraint forms, and Medication Administration Records. * Makes frequent rounds of the Inpatient Unit to assess patients, safety and securing of the environment and cleanliness of the environment and plans for corrective action when necessary. * Provides ongoing supervision to all RN, LPN, and MHW staff on assigned shift of the Inpatient Unit by meeting at least monthly with each RN to review work performance, recommend corrective action and evaluate ongoing progress. * Ensures a comprehensive and accurate communication network between nursing staff and unit management. * Monitors and evaluates all emergency situations on the Corrigan Inpatient Unit. * Monitors assigned employees’ sick leave utilization on a monthly basis to determine if patterns of sick usage have occurred, and counsel employees on sick leave utilization as needed, documenting process as required. * Transcribes physicians’ orders and administers medications and treatments to inpatient clients. Monitor directly and indirectly the effects of medication to ensure staff recognition of side effects. Completes Adverse Drug Reaction Report Forms as required according to protocol. * Functions as Charge nurse on the Inpatient Unit when needed due to the absence of subordinate RN, providing primary nursing care to patients utilizing nursing process and following standards of care. * Meet with patients to complete safety/de-escalation plans to be incorporated in patient treatment in order to provide early de-escalation interventions to reduce the use of restraint. Demonstrates knowledge and skills necessary to provide appropriate care for the age-related cultural and spiritual needs of the clients served. * Ensures all documentation in the medical record reflects knowledge of the patient’s comprehensive treatment plans. _Required Qualifications:_ * * Two years of full-time, or equivalent part-time, experience as a registered nurse in a recognized hospital, clinical or medical facility. At least one year must have been in a supervisory, administrative capacity. * Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing. *_Preferred Qualifications:_ * Experience working with individuals with serious mental illness, behavioral or mood disturbances. * Experience working in facilities that are accredited by the Joint Commission, Department of Public Health, Centers for Medicare and Medicaid Services. _ DMH Mission Statement: _ The Dr. John C. Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination.
