PACT staff will coordinate and provide comprehensive recovery and rehabilitative services to consumers participating in PACT. Interventions include a range of therapeutic & behavioral services, including crisis intervention, for individuals with both severe mental illnesses and/or substance abuse disorders. Services are predominantly community based, including at consumers\' homes, medical facilities, & other locations. Responsibilities: Provide case management services as part of a multidisciplinary team Act as the point person to mitigate housing issues as they arise. Develop treatment plans with consumers. Act as a primary case manager to consumers in accordance with Eliot?s policies and PACT standards. Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner. Assess client\'s functions, strengths and needs. Develop therapeutic connections with each client and provide clinical interventions. Provide problem-solving supports and interventions to clients Provide on-call on a rotating basis and rotating evening shifts. Schedule: Monday - Friday 8am to 4pm, with one 12pm-8pm shift per week and one weekend per month. Rotating on call duties & rotating holiday schedule. Qualifications Education Required Bachelors or better in Psychology or related field. Experience Required 2 years: Experience with adults that have persistent mental illness 1-2 years: Experience in working with individuals with substance abuse issues.
