Actalent Case Manager in Easthampton, Massachusetts

Job Title: Case ManagerJob Description

As a Case Manager, you will be in charge of building relationships with clients and all associated partners such as schools, therapists, agencies, clinicians, supervisors, foster homes, and others. You will develop interventions and set goals for client success in conjunction with their guardians, therapists, and other service providers. Initiating and monitoring external recommendation processes and treatment planning for clients in your caseload will be part of your responsibility. Ensuring compliance with policies and procedures, including all contract sources, is crucial to the role. You will also be required to participate in on-call services as needed and represent our interests in task forces and community boards. The role involves planning and organizing work effectively, identifying problems, evaluating alternative courses of action, and maintaining the highest level of confidentiality. A valid driver's license and adequate personal auto insurance are required for this role.

Job Type

This is a permanent position.

Work Site

This is a fully on-site position in Easthampton, Massachusetts.

Work Environment

The work environment is a typical office setting with common areas and offices. The noise level is generally low to moderate. The setting also includes individual homes with environments unknown to the agency. The standard schedule is Monday-Friday 9am-5pm with potential weekend work if needed.
