Title: Admissions Coordinator ( Nurse RN or LPN )

Join our growing admission team!Seeking admission liaisons to become a member of our growing team in the Dartmouth/New Bedford and Milford/Worcester markets. Offering skilled nursing, assisted living, adult day health program, and Home Care!

Job Summary:

  • Works with discharge planners regarding potential referrals/admissions and program development.

  • Evaluates the clinical information of referred individuals for admission based on services that the Essex Group Management network is able to provide.

  • Reviews and verifies the financial information of referred individuals.

  • Familiar with Medicare and Managed Care regulations.

  • Communicates with all of our entities regarding medical leaves of absence, admissions, referrals and discharges.

  • Experience with online referral systems a plus; Allscripts, Curaspan, etc.

Qualifications:

  • EDUCATION: A nurse with subacute/long-term care experience preferred. Prefer a college degree from accredited college or university. Ability to speak, read and write English. Ability to follow written and oral directions.

  • TRAINING AND EXPERIENCE: Medical knowledge of long term care; financial knowledge of Medicaid, Medicare and other payor sources. Must be proficient in Excel, Microsoft, and Word. Must possess good interpersonal skills, organization skills and be self-directed.
