Eliot Community Human Services PACT General Case Manager in Danvers, Massachusetts
Description

Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation & hope to those in services.
PACT staff will coordinate and provide comprehensive recovery and rehabilitative services to consumers participating in PACT. Interventions include a range of therapeutic & behavioral services, including crisis intervention, for individuals with both severe mental illnesses and/or substance abuse disorders. Services are predominantly community based, including at consumers' homes, medical facilities, & other locations.
Responsibilities:
Provide case management services as part of a multidisciplinary team
Develop treatment plans with consumers.
Act as a primary case manager to consumers in accordance with Eliot’s policies and PACT standards.
Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner.
Assess client's functions, strengths and needs.
Develop therapeutic connections with each client and provide clinical interventions.
Provide on-call on a rotating basis and rotating evening shifts.
Provide problem-solving supports and interventions to clients
Schedule: Monday - Friday 8am to 4pm with standing 12pm-8pm shift. Includes rotating weekends and on-call.

- Bachelors or better in Psychology
