Job Summary
The Home Health Aide provides personal care and light homemaking services to home care patients in accordance with the plan established by the primary nurse/therapist in each case. The Home Health Aide reports to the Rehabilitation Supervisor, Home Care.
Minimum Qualifications
Education
High School diploma and successful completion of the training program required by CMS or the equivalent.
Experience
Preferred
Licensure and/or Certification
Home Health Aide or CNA certification preferred; BLS for the healthcare provider.

Basic computer skills
Ability to maintain confidential information
Ability to read, write, and communicate in English
Excellent customer service skills
Valid Driver’s license and ability to use own vehicle for transportation.
