The Overlook Per Diem - Visiting Registered Nurse Case Manager in Charlton, Massachusetts

Job Description Position Title:             Registered Nurse (RN), Per Diem FLSA STATUS:                     Non-Exempt

REPORTS TO:                     Manager of Clinical Practice

SUPERVISES:                       LPNs, Home Health Aides

DEPARTMENT:                  Home Care

POSITION SUMMARY :

The Registered Nurse (RN), Per Diem plans and provides individualized nursing care and comprehensive services to home care patients. The Registered Nurse assists in clinics and other agency programs as assigned by the Nursing Manager. The Registered Nurse is expected to meet established productivity standards (to include weekends/holidays as needed) as well as a rotation of on-call responsibilities.

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

The following duties are normal for this position.  This list is not to be construed as exclusive or all inclusive.  Other duties may be required and assigned.

  • Assesses patient’s health problems and needs and develops plan of care and updates to the plan of care.

  • Provides direct skilled nursing care as per physician orders.

  • Provides appropriate pain/symptom management. Evaluates and communicates patient’s response to treatments and medication.

  • Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.

  • Documents in medical record and prepares other reports as needed.

  • Educates patient and families in self-care and health management.

  • Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.

  • Provides case management when appropriate.

  • Attends case conference as requested.

  • Adheres to all Overlook and Hospice policies and procedures .

  • Participates in agency quality management and safety activities.

  • Maintains required licensure, certifications and vaccinations.

    SUPERVISORY RESPONSIBILITIES

  • Completes home health aide supervisions and orientations as needed.

  • Co-signs orders and provides oversight of care for LPNs.

    Educational Requirements and Experience:

  • Graduate of an accredited school of nursing - required

  • Current license to practice as a registered nurse in the state of Massachusetts - required

  • Current CPR required

  • One year nursing experience, preferably med-surg

  • Current MA driver’s license and reliable transportation - required

  • Must have working knowledge of HomeCare HomeBase - required
