Job Description Position Title:             Home Health, Registered Nurse FLSA STATUS:                     Non-Exempt

POSITION SUMMARY :

The primary function of this role is to conduct admission and resumption of care assessments with a focus on developing a patient specific plan of care. The RN in this role provides quality and safe delivery of nursing and home health services that meets the organization’s mission of providing exceptional patient care.   

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

The following duties are normal for this position.  This list is not to be construed as exclusive or all inclusive.  Other duties may be required and assigned.

  • Workdays are Friday through Monday

  • Assesses patient’s health problems and develop patient specific plan of care.

  • Provides physician ordered skilled nursing services.

  • Completes 6 points daily which can be a combination of SOC and ROC’s.

  • Assess appropriateness of patients at SOC to ensure they meet the following criteria that are justifiable for services: homebound, identifiable skilled need and focus of care.

  • Has a clear understanding of oasis and plan of care development.

  • Educates patients and families on patient specific plan of care.

  • Collaborates with the physician, home health team members and other care providers, within and outside agency to enhance patient care.

  • Attending case conference as requested.

  • Adheres to policies and procedures.

  • Participates in quarterly QAPI meetings.

    Educational Requirements and Experience:

  • Graduate of an accredited school of nursing

  • Current license to practice as a registered nurse in the state of Massachusetts

  • Current CPR required

  • One year nursing experience, preferably med-surg

  • Current MA driver’s license and reliable transportation

    Knowledge, Skills and Abilities:

  • Language Ability:  Ability to read, write, speak and understand the English language.  Knowledge of the structure and content of the English language including the meaning of words, rules of composition, and grammar.  Ability to effectively communicate with peers, supervisor, residents, clients, and guests. 

  • Mathematical Skills:  Ability to add, subtract, multiply and divide in all units of measurement, using whole numbers, common fractions and decimals.

  • Cognitive Demands:  Ability to apply common sense understanding to carry out instructions provided in written, oral or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.  Able to swiftly change direction as necessary.

  • Computer Skills: Ability to use Microsoft Windows, email and the Internet.  Ability to use software/device for data entry of visit documentation.

  • Competencies: As outlined in orientation plan.

    ENVIRONMENTAL ADAPTABILITY

  • Works primarily indoors in varying climate-controlled settings

  • Possible exposure to unpleasant odors

  • Possible exposure to chemicals as identified in the MSDS Manual

  • Subject to hostile and emotionally upset family members, personnel and visitors

  • May work beyond normal working hours and on weekends and holidays when necessary

  • Required to travel to multiple work sites and up to 200 miles per day (Productivity is adjusted to accommodate travel – every 100 miles traveled in 1 day = 1 productivity point
