Director, Behavioral Health, Clinical Care Partnership is responsible for the leadership, management and oversight of five to seven direct reports BH Managers, who will have responsibility for the day to day management and oversight of Behavioral Health Managers , and direct line BH staff within the Care Partnership teams. The Director, Behavioral Health Clinical Care Partnership provides guidance to the BH Clinical Managers and direct line BH staff and is accountable for the outcomes of five to seven behavioral health teams, ensuring that the teams meet the Key Performance Indicators (KPI’s) set out by the organization. The Director, Behavioral Health Clinical Care Partnership is also responsible for ensuring integration between Behavioral Health and Primary Care delivery throughout Care Partnership. Care Partnership is accountable for the care management, care coordination, and provision of enhanced primary care and behavioral health support to a majority of CCA’s SCO and One Care membership. This position reports into the Vice President of Behavioral Health.

•Provide leadership, individually and through direct reports to meet the clinical and health plan metrics set by CCA Clinical Leadership all while remaining aligned with CCA’s mission and values
•Accountable for KPI’s associated with the Clinical Enterprise: Including lead and lag measures related to psychiatric admission/readmission rates. In addition, ensuring that the care provided directly by CCA BH Managers and BH clinicians is of the highest quality and appropriate for the individual needs of the member with regards to Root Cause Analysis, behavioral health diagnostic assessment, BH HEDIS measures, ACA risk adjustment, crisis management and planning and building strong partnership with outside vended BH clinics and services to ensure that CCA members receive appropriate and timely behavioral health care and support.
•Accountable for KPI’s including support with medication adherence post psychiatric hospitalization, evidence of positive management of behavioral health needs and evidence of management of psychiatric crisis needs.
•Participates in and direct Quality Improvement efforts that pertain to BH and that support other Clinical Partnership Quality Measures
•Accountable for KPI’s associated with member experience: including timely engagement between newly enrolled members and support of their behavioral health needs, standardized frequency of visits, appropriate utilization of CCA’s internal clinical services to decrease acute care utilization
•In coordination with the VP for BH, the HOW Director and HOW Clinical Managers, develop and round out HOW programmatic framework as it integrates with Behavioral Health utilizing evidenced based SDOH measures to meet the psychosocial needs of CCA’s members.
•In coordination with the HOW Directors and Managers, promotes BH and HOW integration within the teams by maintaining close collaboration with multiple Directors of Clinical Services across the Clinical Enterprise
•Provides clinical oversight and support to the HOW Director and How manager as needed
•Engage as needed with cross-functional leaders and external stakeholders including partnering community services, PCP’s, ASAPs, and Independent Living Centers to enhance communication and relationships across the organization.
•Responsible for reviewing and distributing cost and utilization data across teams, as appropriate
•In coordination with the BH Managers, is responsible for the ongoing operations of the BH teams across the clinical partnerships
•Responsible for local oversight/dotted line support of the Psychiatric Nurse Practitioners integration with the clinical teams
•Working with BH Managers, responsible for oversight of TOC with regards to post psychiatric discharge and member needs including working closely with the TOC teams to ensure smooth transition between medical, SNF and community services
•In coordination with the Director of Health Homes Programming and the Health Home Clinical Manager, responsible for the clinical case rounds and review of utilization at the local Health Homes monthly rounds.
•Direct supervision including performance reviews for direct reports
•Works with HR and other managers to hire, retain, and recruit for positions as needed
•Investigates, documents and reports on complaints
•Acts as a liaison and maintains good working relationships with external collateral providers such as ASAP’s, HSP’s, primary care sites, state agencies, and hospital systems and community behavioral and social support systems
•Makes recommendations to leadership on approaches to enhance the effectiveness of clinical practice and programs
•In coordination with the VP for BH and the Director Clinical Partnerships, participates in budget development by forecasting staffing needs, defining space and operational requirements
•In coordination with the VP for BH and the Director Clinical Partnerships, supports budget review for the clinical group, including evaluating variances and creating action plans to prevent further variances
•Manages performance tracking system to ensure performance reviews are up to date and improvement systems are in place, where needed
•Be an available resource and an active participant to support the clinical team/s and conduct home visits with clinical staff, as needed. Be an active resource to triage BH Crisis issues for all staff as necessary
•Identify and foster career development and advancement opportunities for members within the team
Lead the initiatives/activities that the clinical leadership has prioritized
Follow through/follow up, respond to the data/metrics associated with the work of the teams, provide guidance and leadership to meet organizational goals.
Leads formal multilevel direct-report teams and informal cross-functional teams
Gains members' input to and sets the team's vision, goals and strategic plan
Allocates functional roles, levels of accountabilities and strategic assignments
Coaches, mentors and trains new and existing team members in political, organizational, industry/business, behavioral and technical skills
Leads team strategy and planning meetings, conferences, and organization wide meetings
Supervision Exercised Responsible for multiple layers of staff
Master’s Degree or equivalent
Current, in good standing, LICSW or LMHC in the state of Massachusetts
Minimum 5 years related experience
Minimum 2 years of management experience
Ability to function independently and effectively as part of an interdisciplinary team
Strong and effective communication skills, both written and verbal
Strong interpersonal and customer relations skills
Effective teaching skills
Mediation/facilitation/conflict resolution skills
Demonstrates analytical skills necessary to review data
