SOCIAL WORKER, MSW

Job Description:

JOB DESCRIPTION:

  1. Provides clinical consultations and thorough psychosocial evaluations as an integral part of the Emergency Department.

  2. For maximum continuity of care, will follow patients when admitted to MAH. Will provide psychosocial information to the inpatient team for optimal collaborative care.

  3. Guides patients/families through hospital services, accessing needed health and community services, as indicated. Provides patients and families links to a variety of community services.

  4. Participates in community outreach, and maintain connection with local agencies to ensure resources for patients.

5.Cross trains with other social work staff to enable adequate coverage.

JOB REQUIREMENTS:

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED 1. Master's degree in social work from an accredited school. 2. Minimum 2- years experience in health care, with special interest and understanding of mental health diagnosis, substance issues, interpersonal/domestic abuse. 3. LCSW licensure with plan for LICSW. 4. Excellent interpersonal and organizational skills. Strong computer skills. Demonstrated ability to work collaboratively in interdisciplinary teams. 5. Ability to exercise sound clinical judgement in fast-paced and demanding settings.
