Department Description:

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Job Summary:

In collaboration with the PACE Interdisciplinary Team (IDT), the PACE Caseworker in this position is responsible for assisting PACE participants with access to government benefits, including accurate and timely submissions of MassHealth applications for annual reviews; MassHealth conversions to long term care; Medicare enrollment issues; and correcting loss of MassHealth and Medicare benefits. Serving as an interface between participants, families, professionals in the community, and various government entities (MassHealth, Medicare, Social Security), the Caseworker must demonstrate strong interpersonal skills, as well as the ability to apply critical thinking in order to ensure applications are completed and processed by the state in a timely manner. Creative solutions and hands-on assistance in complex situations are required, including visits to participants' homes, banks, local Social Security office, etc. to assist in the renewal of benefits. In addition, they may also assist with government subsidized housing applications and related renewals.

Qualifications:

    • BS in related health field or relevant experience
    • 1 year experience working with the geriatric population
    • At least 2 years experience with care coordination, community outreach, resource coordination, and/or Masshealth enrollment, renewal or conversions
    • Reliable transportation for home visits
    • Flexibility, initiative, creativity, and excellent organizational skills
