Town of Burlington Social Worker - Youth Homeless Case Mgr in Burlington, Massachusetts

The Town of Burlington seeks qualified applicants to perform outreach and administrative work for Burlington Youth & Family Services (BYFS), a Town department responsible for providing mental health and social services to families with adolescents and young adults. BYFS also provides information and referral services for all residents regardless of age. Working under the direction of the Executive Director, the Youth Homeless Case Manager provides case management and housing solutions for youth ages 17-24 living in the Burlington community; conducts intake assessments; utilizes Homeless Management Information System (HMIS); engages with the Youth Advisory Board (YAB); interfaces with Town and community agencies; provides referrals and advocacy to stabilize the household and support financial independence; keeps timely and accurate documentation; as well as other related tasks. Job Responsibilities: Work with community agencies to identify youth ages 17-24 who are at imminent risk of or are currently homeless Engage the identified youth in a coordinated entry process that emphasizes housing first Support youth in stabilizing their income and lifestyle in a way that supports permanent affordable housing Become familiar with and implement all requirements of HUD/DHCD grant award Utilize input and participation from Youth Advisory Board throughout the grant period Refer youth to services and benefits that will help support on-going sustainability of their household Bachelor\'s degree in a social work or counseling field is preferred, as well as 2 years of experience working with transition age youth and/or homeless population. Equivalent lived experience will be considered a valid substitute for education. Driver\'s license is required, and regular access to a private vehicle is preferred, although there is a company vehicle available at times for the use of the Social Worker. This is a grant-funded two-year position, with the possibility that it will be extended in 1-year increments if the program outcomes are evaluated as successful.
