Senior Clinical Pharmacist/Specialist - Hematology/Oncology

Job Description:

The Senior Clinical Pharmacist/Specialist for Hematology/Oncology works collaboratively with other health care providers to provide patient-centered care, with the goal of providing evidence-based, cost-effective pharmacotherapy for Lahey Health patients. The scope and nature of patient care services provided will depend on the practice setting, but typically would include active involvement in interdisciplinary patient care activities, pharmacotherapy consultation, medication therapy monitoring, provision of drug information, education to both health care providers and patients, and guideline development and implementation. Additionally, the Clinical Pharmacy Specialist assists with the continued development and promotion of clinical pharmacy services by participating in pharmacy staff, resident and student education, as well as support the safe and effective use of medication through Pharmacy and Therapeutic Committee involvement.

Essential Duties & Responsibilities including but not limited to:

Quality Focused Activities:

  • Promote Pharmacy involvement in the collaborative care of Lahey Health Patients. Promote Lahey Health through participation in professional organizations or meetings and conferences.

  • Utilize practice management skills to maintain cohesive, cost-efficient and productive care of patients and relationships with other clinicians.

  • Participate in formulary management and review of medication use processes through involvement.

Current MA Pharmacist License:

  • 2 Years experience in Hematology/Oncology
