Enable Networks Supported Living Service Direct Support Professional in Brockton, Massachusetts

Full Time - Part Time and Temporary Positions Available Position Description/Minimum Qualifications * Job Overview Networks Supported Living Services is a program of Enable, Inc designed to provide case management to individuals with disabilities allowing them to live independently in the community. Persons served are consumers, rather than clients and services focus on self-reliance and self-determination as the cornerstone for success. The program is funded through Massachusetts Rehabilitation Commission, Department of Developmental Services, and MassHealth. The MRC Direct Support Processional (DSP) provides additional supervisory and hands-on support for consumers for needs not covered by the MassHealth PCA Program. DSP employees may work for more than one consumer. Duties may vary from consumer to consumer and will be directed by the consumer and the Supervisor. This position requires someone who is highly ethical, self-motivated and detail oriented. Responsibilities and Duties - Provides supervision and safeguards for consumers as needed. - Performs such household chores as shopping, laundry, or cleaning the consumer?s apartment or bedroom;care for and maintain adaptive equipment. - Supports consumers in engaging in social and community activities, as requested by the consumer anddirected by the Case Coordinator; support consumers in personal activities at home or away from home asrequested and directed. - Accompany consumer to medical appointments, provide Case Coordinator with appropriate paperwork andensure follow-up is completed on any necessary medication changes or other needs. - Accompany and stay with consumer upon Emergency Room trips; brings all required paperwork andcommunicates with Case Coordinator immediately. - Assist with scheduling transportation, communicating with collaborating agencies as needed. Reports backto Case Coordinator at all times. - As needed, sets limits and redirects consumer. - Completes written notes as assigned. - Maintains confidentiality in all consumer-related matters. - Maintains professional relationships with all outside agencies. - Seeks support, supervision and direction in a timely and professional manner; notifies supervisor or othersupervisory staff as listed below immediately in case of any urgent problems or situations. Qualifications - Familiarity with population served or related population. - Familiarity with ethical and other considerations in the provision of services in the homes of individuals. -FA/CPR required - Must be able to lift without restriction
