Old Colony Elder Services (OCES) Behavioral Health Clinical Social Worker in Brockton, Massachusetts
OCES aims to support elders and individuals with disabilities by providing vital information and coordination of services that promote healthy, safe lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities for as long as possible. And we are growing! The Behavioral Health Team at OCES is seeking Clinical Social Workers to support our consumers in the community using a person-centered approach to address behavioral health needs. What you\'ll do: * Work directly with older adults with complex behavioral health conditions who require long- term case management and/or counseling to remain safely in their community. * Conduct home visits as needed to assess and develop comprehensive, person-centered care plans with consumers, caregivers, and families. * Provide advocacy, support, and assistance to address unmet need including but not limited to housing, hoarding, substance abuse, medical case management, and communication with providers. * Conduct outreach to local providers about Behavioral Health services. * Coordinate and/or facilitate support groups based on community need. What you\'ll bring: * Master\'s degree in social work or related field; current LCSW/LICSW license preferred (or ability to sit for the license. License differential paid. * Three years\' clinical/community-based experience. * Extensive knowledge of the psychological, emotional, and social needs of older adults and people living with disabilities. * Great assessment and communication skills * Strong ability to work independently. * Bilingual highly preferred; Haitian Creole * Proficient with Microsoft 365 and other software.
