Friday, with most Saturdays needed Individuals who identify as immigrants or limited-English speakers can be some of the most marginalized members of our society, especially if they are experiencing homelessness. In response to this inequity, BHCHP developed an Immigrant Health Team to address the health care needs of immigrant individuals and families who experience homelessness or unstable housing in Boston. To develop the program further, we are hiring a Program Coordinator who will be responsible for leading case management efforts impacting immigrant patients experiencing homelessness. They will coordinate with other case managers as needed to assure all immigrant and LEP patients? needs are being met, and that there is no duplication of services. This person will also engage in outreach and recruitment of patients, event management, foster empowerment including linkages to support groups, and participate in community advocacy efforts to decrease disparities in health care for this population. Responsibilities: Build relationships and collaborate with multidisciplinary teams of providers and nurses to provide and assure comprehensive case management services to homeless individuals Work with clients to obtain the full range of benefits and services to which they are entitled, including housing, primary care, social and psychiatric services, and all other necessary services Present about the Immigrant Health Team at monthly new employee program orientation, provide resources, and advocate for inclusive practices at BHCHP and our community partners Collaborate with external organizations to create support groups for clients in both English and Spanish and assist with facilitation; also co-facilitate existing BHCHP multilingual support groups; collaborate with AmeriCorps member(s) who are part of the Immigrant Health Team on projects, including outreach/recruitment and community event planning Participate in community advocacy efforts that BHCHP is already a part of, such as the New Arrivals Working Group, Office of Refugee and Immigrants (ORI) Working Group, and others as appropriate; lead implementation & coordination of QI activities at team level; monitor team performance & lead team QI efforts Serve as a resource for the BMC Preventive Medicine Fellow(s) in immigrant and refugee health who will join Oasis every spring/summer and working at the intersection of immigrant and public health Collaborate with the Director of the Oasis Clinic to shape overall direction of services and goals Oasis Clinic Saturday Clinic Regularly attend and be the on-site lead for Saturday Oasis Clinics Oasis Volunteers Help recruit and onboard new Oasis Clinic Volunteers Support current volunteers through regular meeting updates / communications Management of volunteer roster to identify current needs and capacity Schedule support for Oasis Saturday Clinics Coordinate BHCHP Asylum Clinic Collaborate with and facilitate referrals from peer asylum clinics in Greater Boston Recruit, coordinate orientation, and support forensic evaluators Utilization of funds from BU Refugee Challenge Schedule and track evaluations (and their outcomes) SIREN grant (2023-2026): attend grantee check-ins and provide updates for this newly formed Community Advisory Board for asylum clinics Case Management Provide immigration-specific case management services and consultation support for other CMs case managers in non-duplicative manner Provide training support to BHCHP case managers, and other staff as needed, to improve the program?s overall capacity to serve LEP patients experiencing homelessness Attend and lead bi-weekly immigration case conferences Attend immigration-related community-wide meetings and trainings assist in grant-writing and research opportunities Qualifications: BA/B preferred, es
